Have you set the display currency code and paypal currency code to the code for South Africa from the Payment Settings tab under the "Manage General Options" page? If so that should replace the $ with the code for the South African currency. Note that the code and the symbol are treated differently. The plugin only recognizes the standard symbols so in the case of the South African currency it would not replace the $ symbol with your "R" symbol, it would replace the $symbol with your currency code "ZAR".
